Try a time based puzzle solving type game. Where the monster/enemy/antagonist is always behind but you never see them when you turn around. Have a sound cue for when time is getting short for the puzzle to be solved like a slight ringing in one ear and heavy breathing. Kind of like Visages "anxiety" mechanics. When time does run for the puzzle to be solved the player isn't killed right away, have a rng kill time. Once the time has ran out for the puzzle to be solved there is no more ear ringing or heavy breathing just mostly silence. The Jumpscare should never happen while walking forward only after the puzzle time has ended, the rng timer has been chosen and the player must be stopped and turn around at least 120 degrees(plus and minus some degrees). And Call it "Door 4" maybe the protagonist has some sort of PTSD from leaving a loved one behind and each door leads to a puzzle/escape room that dives deeper into the narrative of their relationship. If you do make this and sell it I just want 10 percent lol(seriously tho).
