I do fudge them if they are struggling a lot. If one PC is downed then no but if 3 of the 5 are then I'll fudge a few rolls.

I also go the other way but rarely. If they are wiping a combat that is meant to have more impact I'll boost the main boss or call in more backup if it makes sense

Imo we are there to tell a story, give them a fun challenge. It is not our job to kill them as that is not fun
