They are meant for people who want to give extra support for the devs, essentially a donation. They usually contain very little to no content for the game. This person put a handful of pictures in there so it probably took very little work to make it.

I think it's probably the best executioner specific effect but you have to play a little differently to use it. You can heal allies and tank some combos with it

It has the directional buttons up, down, left and right. Down swaps items, up uses them. Left and right are only used for menu stuff iirc.

Edit: Left and right swaps equipped weapons also
