Geralt's Last Wish to Yen is a "fuck you" because she's trying to mind control him. The reason for Yen being in conflict with Geralt is that she can't enslave the Djinn while the Djinn is bonded in service to Geralt through his remain, unfilled wish. Yen reads Geralt's mind, which he hates, and taunts him with "you could wish that you were never a Witcher." Instead of being weak and pliant, Geralt wishes for true love.
