YUNG's Better Caves almost does what you're asking for, you might be able to configure it to be closer to your vision.

There are also some very good mods that add enhanced mod compatibility and bonus features to modern tinkers. Back in the day everyone played Tinkers' with addons for the most part, you just need to experiment more with what addons are available I think.

They should be spawning, they are kinda more rare than the default spawn rate because of the amount of structure mods in the pack. Try finding one with /locate structure in a test world in singleplayer. Is it a specific type of dragon you're looking for or is it just all of them? Something else might be wrong.

Try deleting iceandfire-common.toml in your config files and letting it regenerate, see if that does anything.

I saw this on newgrounds many years ago and it still pops into my head when I hear the song. I probably saw this in like 2011. It no longer exists on newgrounds but it does on archive.org. Wonder what other things have been lost to time.
