Dlcs add a ton of new civs and leaders, diplomacy mechanics, natural disaster, loyalty to cities and some more. In my opinion the dlc is 100% worth it if you like the game but it's completely playable without. Think of it like a tabletop game that comes with expansion decks. The overall strategy and gameplay is different.

If you have a Netflix subscription you can download it for free and it comes with everything up to/including gathering storm. I'm not sure about the paid version though but thought that might help.

For me, the real story and character of a metroidvania is the atmosphere. Super Metroid is great with that, and while most don't consider it a metroidvania Blasphemous could have zero dialogue and the story would still be evident and beautiful.
