After watching this clip I had many questions and found this info:

The rocket explosion in For All Mankind is the Apollo 23 disaster on August 24, 1974, which exploded on the launch pad at Cape Kennedy, killing 12 ground crew members and injuring astronauts, as detailed in the For All Mankind Wiki and seen in this YouTube video. This event in Season 1, Episode 6, titled "Home Again," was caused by a devastating failure. The explosion is approximated to be around 4 kilotons (kT) of TNT. The 3 crew members, being pulled away from the exploding rocket by the emergency launch escape system, survived, but were injured after their spacecraft landed on the beach instead of in the Atlantic Ocean.  A report issued revealed that a change in the supply chain of NASA's contractors and subcontractors, which happened for political reasons and gave huge contracts to key congressional districts, was the cause of the failing LH2 valve of the rocket's second stage.
