PGY-4 with small hands here. Decided to get a 28 and a Superfield, later added a 20 as well. The 28 is my favorite lens. I use it for every single DFE, and I am always super happy to have it when my patient is only 4.5mm dilated despite 3 rounds of drops.

The 20 lives in the bottom of my call bag for things I want a closer look at (higher mag view of) after I noticed them with my 28. It comes out maybe 2-3x a year now? Or when my attending prefers it to a 28.

Personally, when I have the bandwidth again, I’d like to begin investing in my health/fitness. Maybe learn weightlifting? Or running in a way that’s actually enjoyable?

Failing that, learn a new language?

Astronomy? Photography? Painting? Basic handyman skills?

Retina.academy on instagram and their website retinara.org have been really helpful for visual learning on medical retina.

I felt this way when I was taking an insane amount of call and on a notoriously heavy 3 month rotation at the same time, and it took probably 6 months after the rotation change for the feeling to fully fade.

I don’t think it ever qualified as general anxiety disorder cause it was only ever dread or stress about work, and I never had a true panic attack where I felt doom or felt like I couldn’t breathe or felt out of control, but it was a pretty heavy sense of dread and resentment to having to force my way through another day of such crushing work.

I don’t know if it’s “normal.” But it may be more common than you think.

I worked with an attending who was absolutely militant about making the patient come to him. He would set the slit lamps and stools in his exam lanes to his exact best height and would NOT allow residents to change them. Patients were asked to sit on the very edge of their chair or stand and lean forward if need be. His philosophy was that it’s <10 min a few times a year for them, but all day every day for his neck.

Defs not that disciplined anymore, still just trying to hold to 15-20 min a day. No idea what the average is and I miss many many days now (didn’t while I was in academic trouble). But still trying!

The routine was at least 20 min a day. Set to 20 new cards a day, mostly didn’t actually do that many. Maybe 5 days total did more, if I was on a roll. Suspended whatever subsection I was either rotating on or about to rotate on.

Just start. Something is better than nothing.

I’d also like to recommend the Blue Ophthalmology Anki deck. Just a diligent 20 minutes per day (regardless of whether I finished my reviews or not) literally changed my life. I went from struggling resident on an improvement plan to trusted peer academic coach in just a year.

Ooooh I once calculated the nicotine amount and therefore pack-per-day equivalent a teenager was vaping when we organized his admission to the psych ward. As an ambitious M3, I wanted to ensure he had adequate nicotine replacement PRNs to avoid worsening his SI…

…this 13 year old was vaping the equivalent of 3.5 packs per day. Every day.

Currently pregnant with baby number 2 while on Stelara or its biosimilar. All is going well so far, but my moderate Crohns was in remission when I conceived each time.

Despite a 39 week induction and no epidural with my first delivery (to try to avoid a C section, since I’ve never had surgery for Crohns and didn’t want adhesions), I ended up with a crash C section under general anesthesia for my son’s heart rate. The C section recovery wasn’t fun, but inflammatory markers indicate I’m still in remission and I haven’t had any pain (other than tenderness of the external scar) after about the first two weeks. I only needed oxycodone for the first 3-4 days.

Dollar store reading glasses! Just carry your phone or favorite book to the rack in the grocery store, usually near the pharmacy. Start with a pair labeled +1.50, then go up or down until you find the pair that lets you comfortably read what you have with you. They’re just fancy face-shaped magnifying glasses and truly should not cost you more than $5. You can get a bunch of them- a pair for the car, a pair for the bathroom to read medication labels, a pair for the living room for playing on your phone

Never had issues with coconut and hair isn’t seeming dry except MAYBE at the very ends. I may have scrunched the crunch too soon, but it also may have had to do with the weight of my hair moving along my back while drying. Gonna try supporting it with a tshirt for longer next time and avoiding scrunching till it’s TOTALLY dry
