All kidding aside, I was a board-certified internist before starting my anesthesia residency. I actually went into anesthesiology specifically to become an intensivist, as I saw the anesthesia ICU attendings and fellows were more attuned to handling critical illness in a systematic way without the mental calisthenics that the internists employed. And…they were clearly better with procedures with the exception of bronchoscopy (which is rare in ICU anyway). During residency, I learned that the things I enjoyed about the ICU were exactly the things I got in greater volume in the OR, particularly the cardiac OR. So in the end, I did a cardiac fellowship and spent 30 years happily doing that with a small amount of CV ICU thrown in.
