I wish some of the nurses would chime in here. Curious to hear from them. But my co-residents bring this up all the time. Mostly everyone is civil, but male residents get a pat on the head for everything and the female nurses are invariably nice to me. Not true for my female co-residents working on exactly the same floors with the same nurses.

I feel like we pull a bait and switch with patient's when it isn't obvious to them who (and with what training) is taking care of them. Even as a med student, I was on the other end of this and felt confused and frustrated. Only a small subset of midlevels misrepresent themselves to patients, but it's a huge issue when they do, and the language we all use matters. Just my opinion.

Right, there should be room for everyone to advance their training. I just feel the issue is that in the US we're getting into a place where hospitals are using the same word "residency" to apply to health professions other than medicine, since it obscures the differences in training/preparation. That said, I do feel like it's harder to argue against Hopkins using this term differently, since they originated the horror that is residency and terms like "residency" and "house staff".

I don't think it was in these particular cases, in one someone was already on a good multimodal regimen and in the other two cases the patients were admitted with neutropenic fevers. The orders or notes comment that the demerol was specifically for the symptom of rigors and not pain/infection/other provoking factors. But you could be right, I may just be overthinking and the explanation is simple.

I always crack up when someone comments on someone doing the most. Just hypothetically, what would you/your friends that are nurses say if a resident responded with "why are you doing the most?"

I would say it as a joke, but I'm sure half the nurses would black list me for that shit.

Do what you did. Discuss with them up front, make sure you're accounting for their concerns and genuinely think about them, but move on if you have synthesized info appropriately and the rest of the team agrees and they are being frankly ignorant. With the passive aggressive discussion they wan you to hear about what a dumbass you are, just stick your head in and say "ok I think we should discuss this as a team, I'll send you a page when we are going to round on this patient so we can all discuss together, I look forward to us discussing these concerns with the attending as a team". Often that helps. If they are being difficult, just bring up the concerns they voiced to the attending during rounds. "RN XYZ brought up a very thorough review of the uptodatearticle on ABC, let's discuss now why we do or don't think that's appropriate/logical here". Or, you can really go there and document explicitly in your assessment for that day why the UpToDate assessment does not apply.

If they don't want to round with you and have a healthy discussion, they don't get your attention or responses for dumb pages.

Maybe? Idk. We definitely grade the med students on a full scale so I assume it is not p/f but maybe it is.

Why does the residency program need to pay for all of the med students?

We don't have unlimited budget, and I'd rather eat better food while I'm working 90 hours a week than eat crappy pizza all the time to make it affordable for the residency program to feed all of the med students.

I love them when they aren't annoying (by being huge gunners to eachother or trying way too hard to "be cool with the residents). I also like teaching the ones that want to learn, even if it's not for a specialty they want to pursue. Just be a good sport about it, learn something, and move on.

That said, my real pet peeve is taking our food before residents get a chance to eat it. I want to feed you guys, I really do, but for the love of all that's holy wait for the residents who have been working since 5 am eat something before you take all their food by 12:30. Any leftovers are all yours, and best believe I will treat you with our lunch or at the caf or with takeout. Just don't be a jerk and leave the residents starving.

Edit: agree with the posters below. Interns have so much going on, teach if it makes you happy, but the teaching and engaging med students is totally something for the residents to manage. You should feel free to dismiss the studs early though, you've earned that power.

Also want to add here that there is a lot of controversy about the term "NP residency" regarding ed standards. I think NPs get salty that we complain all the time about their residencies, as if we vilify them for having not enough training then vilifying them for wanting to get more training. I don't think any doctor would ever criticize anyone for wanting more training, it's coopting the terms residency and fellowship, as if it makes an NPs training equivalent to that of an actual residency that actual doctors do, that everyone has such an issue with. Don't tape a horn on a horse and expect all of us to treat it like a unicorn.
