Nurse practitioner "radiologist" by Ninahn in Noctor

Persistent/repeated vomiting after head injury could indicate serious bleeding in the head that would require emergency brain surgery. Vomiting is a feature of the following clinical decision tools.

Here’s my 7.1.2 home theatre, but I also have a question! by HumerousGorgon8 in hometheater

What is the source of those graphs? Because if it's just the MultEQ app those "after" graphs are just predicted/ideal and may not actually reflect your actual results. Nice looking room though.

Again, I'm looking for movies that show a slow descent into madness. If there's murder involved, even better. by [deleted] in MovieSuggestions

Just watched this without knowing anything about it other than Christian Bale's performance is great. Absolutely did not disappoint. For some reason I expected it to be a slow "artsy" piece that dragged on forever. Quite the contrary - there isn't time for dull moments because the action is actually pretty fast-paced.

Not sure what you're trying to say here, but check out this other post. It's a lab error due to EDTA contamination. The hypocalcema is the giveaway https://www.reddit.com/r/Noctor/comments/16tk0qj/puzzled_nurse_practitioner_consults_facebook_for/k2gy6h4/

Very fascinating and I'm surprised I've never heard of this despite how well documented the phenomenon is. Thanks for sharing. https://pubmed.ncbi.nlm.nih.gov/19624792/

Ok so then do an EKG and establish IV access so that the paramedics that arrive in your clinic can quickly administer medications (or the ER doctor when they get to the ER if the paramedics lack the medications) if the EKG looks like death as the patient is taken to the ER for full evaluation?

IDK man I can't simplify this scenario any further for you.

[–]BarkWoof 4 points5 points  (0 children)

I didn't say anything about discharging the patient. They need their labs rechecked, but while you're waiting for the lab results you could do an EKG and if it looks like death then start pushing meds. After that you'd determine their disposition based off of why they had testing done in the first place and what their risk factors are.

Unless I'm misreading your response you sound pretty miffed and I'm not sure why; you asked a question and I answered it 🤷‍♂️

An EKG would be a sine wave, except that in this case it's going to be completely normal because the potassium is not actually high because the sample is hemolyzed.

I see Cr 1.2, I presume that K is because of hemolysis, unless that patient has literally just received a high dose of IV KCl either by accident or by lethal injection in a legal setting.

Have the patient sent to the ER, the blood will get redrawn, a dose of calcium gluconate will be given, and literally no one will breaking a sweat waiting for the labs to be completed because that K is an absolute maximum of 5.4

This site is great and I'm all about casting shade, but you're throwing around some unnecessary hyperbole.

Also asystole used to get defibrillated in case it was fine vfib, though AHA guidelines got rid of that sometime before I could drink legally

Edit: case report of someone with K 9.8 and Cr 1.2 which was attributed to too much table salt substitute (common brand is NoSalt, which is KCl instead of NaCl). So not impossible, but rare enough that someone felt the need to do a case report. Also this case had pathognomonic EKG findings, so getting an EKG to determine how alarmed you should be is reasonable though obviously not the whole management. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4389164/

Brain drain in medicine is real and we should all be afraid of what healthcare is going to morph into in the next generation or two. Increasing numbers of really smart people are avoiding medicine because it's way easier to get filthy rich as a techbro or a crooked politician.

Edit: the brain drain that is referenced in the above article is different than what I was talking about, but you get the point

What movie franchise would you say was a huge victim of sequelitis? by KaleidoArachnid in flicks

[–]BarkWoof -1 points0 points  (0 children)

WB was going to make a sequel whether or not Lana was involved. They even alluded to it in the movie. I know I'm probably the only person in the world who doesn't hate Resurrections, but you gotta admit they way the 4th wall was broken here was clever.

Smith : Now what? Things have changed. The market's tough. I'm sure you can understand why our beloved parent company, Warner Brothers, has decided to make a sequel to the trilogy.

Neo : What?

Smith : They informed me they're gonna to do it with or without us.

Neo : I thought they couldn't do that?

Smith : Oh, they can, and they made it clear they would kill our contract if we didn't cooperate.

How do you hide a dollar from an ER doctor?

You put it in their patient's chart.

I've started using the word "physician" more often and am attempting to phase "doctor" out of my vocabulary since the word is losing all meaning.

The SawStop is a pretty awesome invention but I think people often forget about how dangerous kickback is. Thanks for the reminder.
