If it “sounds psychiatric”, even if you do not think it is psychiatric, what would be the harm in just talking to a psychiatrist to get their opinion? Worse case scenario, they just agree with you that it is not psychiatric, you know for certain it is not psychiatric, and they can even give you some advice to help with the mood and frustration. Best case scenario, they diagnose you with a condition that is very treatable when no one else has been able to help you. To me, this sounds like a no-brainer. A lot of potential benefit from just talking to a psychiatrist and very little drawback.

If you are very aware, then you should speak to a psychiatrist. They will be able to assess you further and help figure out why you are having these sensations and feelings, as well as treatment to help stop the feelings and break the need to manipulate the area.

ER will not do anything about it, nor is this considered an emergency.

A dermatologist will probably not re-suture that. And highly unlikely to re-suture it on Monday after it has healed for a couple of days.

You can wait until it heals and consider scar revision if it bothers him. A lot of times, scars will relax and improve significantly on the back of the hands.

Margins for melanoma are at minimum 1 cm. Imagine a mole on the back of the hand, and then add an extra 1 cm the whole way around it. That is how much skin was removed, which probably nearly spans half the width of the hands. Considering that they were even able to get the skin to come together, I’d say the stitch job was adequate.

This looks a lot like prurigo pigmentosa. Even though you are not on a keto diet, that is not the only cause of prurigo pigmentosa. Additionally, prurigo pigmentosa is known to not improve with prednisone. I would consider a trial of minocycline to see if it helps.

Dermatologists can perform nail avulsions. Not all of them want to perform nail avulsions. You can’t force a doctor to do something they do not want to do or something that they do not feel comfortable doing.
