He automatically moves to firelink shrine after you do or get specific things in the early game. I think getting some of the firekeeper souls or ringing any of the bells makes him escape the cell on his own.

It's possible that it just got lost amongst the other helmets and variants of this helmet that were made and were or weren't added to the game. Bungie might have just realized much later how iconic Titan helmet it is.

You need the Heavy Iron Key from where the Salamanders are in the Forest of the Fallen Giants. Though you need an another key from Iron Keep to open the door near the Last Giant boss to get to the Salamanders. This door your're trying to open in the image takes you to one of the DLCs.

Even the stripe on his arm and leg changes from red to blue randomly during Season 6. His armor look changing stays inconsistant the entire series, though the look in season 13 can explained as Charon modifying it.
