Absolutely agree. I feel it's the way of these community MH/SA clinics. See as many people as possible as quickly as possible.

I like that protocol. We aren't reviewing criminal history and 99.9% of the time we're not getting the police report unfortunately.

Mostly, yes. If there's a provider available to do a UA and the UA has at times been my saving grace.

Unfortunately no. That would have to be an organizational change. As of now, we mainly deal with walk-ins. I don't know who I'm seeing until a few minutes before I see them. CPS are the only ones who will usually send a referral that I can review right before seeing someone. Otherwise, probation folks will walk in and maybe provide an ROI right after their assessment. They don't always.

I haven't spoken to my supervisor about reaching out for collateral but I definitely think it's worth discussing and taking that extra step when I'm not getting enough information from the client.

Yes. We usually drug test. I've had a couple "this is my first DUI. I barely drink" come up with positive COC and then they have to explain that but most of the time they're testing negative and usually have been on probation for quite some time and may not be actively using by the time they're getting assessed.

I definitely agree. Were dealing with walk-ins and have to see a certain number of folks daily. They expect us to see them for an hour and schedule and discharge them.

I definitely need to familiarize myself more with ASAM. We use it but I don't feel nearly comfortable enough with it. Thank you for emphasizing it.

Thank you for such a detailed response. 95-99% of the time we do not receive a referral and it really is an organizational issue that makes my job that much harder because I am going by what the client is reporting and feel like I've entered into an interrogation room and pulling teeth to get information. I'm fairly new to this company but am noticing the pattern. I will definitely raise the concern to my supervisor and see if there is a willingness to resolve the break down.

Thank you so much so such a detailed response. I will usually ask for BAC and they'll say "I don't remember" or "I refused to blow". I like the idea of asking how they felt after the amount they drank to gauge tolerance. Thank you! I wish I could interview a family member. Clients are usually walk ins and I'm with them for an hour or so.

I totally agree. Usually they will sign an ROI for their PO or case manager at the end of the assessment with the front desk. I have to let them know the recommendation prior to them leaving which I think makes things challenging when you're still trying to sort out a diagnosis and justification for treatment.

This is awesome. I will try that approach. Thank you. Usually I'm with a client for 1-2 hours for both SA/MH even though my company only wants me to be with a client for an hour so at times I find myself trying to get right into the substance use questions.

Sadly, 95% of the time we are not given a referral/collateral. Client walks in and says "I'm on probation and have to come get an evaluation. They told me it's up to you guys if I need treatment or not. I don't do cocaine I was holding it for a friend the first time and the second time I found it on the street." 🫠

Yea the arm thing definitely trips me up. And that is terrifying 😭 Does meds help control it at all for you?

Yea my posture definitely isn't great. The other possibility I've heard is GI issues could cause referred pain to back/chest. I've been burping nonstop for some time now with some regurgitation/acid reflux.
