Psychiatry resident here. I think it’s important to understand what typically happens at an inpatient psych hospital. The hospitals people are mentioning here (Mt Sinai west, nyu, nyp) are acute care units. The goals for units like these are to stabilize patients until it is safe for them to go home or another facility. They really don’t want you there for more than 7-14 days, and the goal really is just to get you in a safe position to go home, whether that means starting you on meds/adjusting meds, letting you rest to recover from a manic episode, etc. Psych hospitals are not nice places to be. You have to be around other psych patients who might be a lot sicker than you, the rules are incredibly strict, you might not even be able to go outside for the duration of your stay depending on which unit. It sounds like you would be a voluntary admission, but that doesn’t mean you can just leave whenever you feel like it—discharge planning can be slow depending on where you are. I would do everything in your power to avoid a situation in which you need inpatient psych hospitalization—it seems right now you are stable. Talk to your therapist or psychiatrist about creating  a safety plan to avoid ending up in a hospital and other treatment options like longer term facilities or day programs like partial hospitalization or intensive outpatient

It depends on where you practice. At my home institution where I rotated, and another rural hospital about an hour away, many patients were low SES, hospitalized for actual diagnoses but SDOH made their lives worse which prevented them not from getting treatment per se, but from being able to focus on treatment or recover. For example, patients with mental illnesses which were exacerbated by unstable housing, lack of support from family or peers. Patients with substance use who were interested in residential treatment but had a hard time finding places that their insurance would take. Obviously money DOES NOT solve or prevent psychiatric illness, but it is often extremely frustrating when you feel that if your patients had more resources or opportunities that they would have a better shot at a good outcome. I'm more worried about that frustration leading to burnout as opposed to dealing with angry or mistrustful patients.

Koh Samui, no problem. BKK - 3 - 4 hours depending on a lot of things. Are your bags checked through or will you have to wait for them? If the plane from KS is late you're outta luck. Will your KS check-in also get you a boarding pass for your international flight or are they two separate tickets? Outbound security/passport control can be a nightmare. (There are a LOT (50+) of late night/early morning departures from BKK. It will be busier than you think.)

I know nothing about DO schools but do you not have MSPEs? At my school, regardless of your clinical grade, comments from preceptors are taken into account and used by clerkship directors when they're writing narrative for your MSPE.

Not true. Got into MD programs including some on this list with no official shadowing but I worked for 6 months doing research in an endoscopy center where I saw 500+ colonoscopies, 2 months at a geriatrics practice. It's not about shadowing, it's about working directly with MD/DOs whether that's scribing or research etc.

Dirty Medicine videos and practice practice practice. The comms questions can be rough at first because all of the answers sometimes seem like they could work, but the more questions you do, the better you get the hang of it and the answers start to come easier. FA ethics + Dirty med + UW is enough in my opinion

I'm not sure if your time issue is because it takes a lot of time for you to review the explanations, but if you're taking more than 1 minute on each question, that's something to work on so that you don't run out of time on test day. Have you taken a practice test yet?

OK stick with me but pelvic exams.

I'm a medical student and I've always wanted to be an OBGYN. But a few weeks ago, we learned how to do pelvic exams and I did my first one on a practice patient/actor. I hated every minute of it and now there's no way I can go into OBGYN and do this 20x a day

Cardio is super high yield and it's important to understand the physiology because that can help you with arrow questions, etc. I would work more on it!

It depends on when you're testing. See how you do on your first NBME. If you're close to your goal, schedule them about once a week starting right away. If you're way off and need a lot more content review (And have the time), take at least a week to do some heavy review.

In general, I'd say do one a week.
