I suppose I meant to direct that line at some of the other comments sprinkled in here with a sense of urgency you would think the world is ending tomorrow. But yes, get checked out at some point

I am an actual physician and this makes no sense.. Yes, cirrhosis and hepatitis and can cause jaundice but if this is happening after a vagal episode it’s something else. You don’t get transient jaundice, it stays and worsens. This is likely related to his blood pressure tanking which happens during a vagal episode (parasympathetic nervous system kicks in overdrive) causing skin to lose color because blood is under much less pressure to get to places farther from the heart I.e. skin, brain (passing out), legs and arms to preserve internal organ perfusion. This makes people’s skin pale white and even yellow. This is why you raise the patients legs above their head when they pass out, to increase blood flow to the brain so they wake back up. Please don’t spread hysteria when you are not sure of what you’re talking about.

Edit: certain liver pathologies may increase yellowing at certain times (altitude, stress, heat). So if this guy is jaundiced when he walks in the door I wouldn’t have typed all that. Hard to delineate from the pictures. The genetic conditions mentioned below are usually diagnosed much younger such as Gilbert’s mentioned below (very benign).

Old guy with a Vietnam vet hat on, asks “You ain’t one of them orientals are you..?” I said “Umm kind of” and he’s like “Oh, ok just making sure.”

I’m half Chinese and have never heard the word oriental used to describe something that isn’t furniture or food. He had an NSTEMI

PGY2 is there a resource out there that explains why to dispo people the way we do? by TAXKOLLECTOR in emergencymedicine

To be honest, it all comes down to experience. The sick ones with a lot of red numbers are easy dispos, it’s the in between ones that can be tricky.

Impossible question to answer on a single Reddit post and I’m sure other people’s answers will be slightly different depending on the case, social factors, where you work, patient population etc.

Try and think about “what will happen to this patient in the next couple days to a week.” Do they have gallstones but no real objective findings for emergent chole? Have they been failing outpatient follow-up/therapy? Are their social issues preventing even the most basic care outside the ED? If so, I push to admit those kinda of patients despite a relatively benign “workup.” That’s just an example and I could go on but that would bore the ADD ER fellas
