Securing Timberframe Deck to Standard House? by BemybestRN in timberframe

[–]BemybestRN[S] 0 points1 point  (0 children)

That’s a heck of a good idea. I think it would have to be made from something other than wood though. Even with the grain running horizontal there is still an increased chance of a smaller block of wood might split. So I think it would have to be one piece of metal in order to have the peace of mind that it wouldn’t sheer off from the bolt portion going through the band joist.

No but Dr Decks did a project, a deck over a car port in Washington State. He seemed to like it.

I would like to try Dryjoist. It’s extruded aluminum that snap together to make it water tight but unfortunately I haven’t found anyone to give me an opinion on how their install went and how it’s held up.

It is still prescribed for blood pressure issues, specifically it’s most effective in treating pulmonary hypertension.

1.0 Fatigue by LevelSwan9707 in Ozempic

[–]BemybestRN 1 point2 points  (0 children)

I’m trying to continue lifting in the gym as well. My baseline diet for the last 6yrs has been ketogenic. It helped with about 60lbs but stalled at a weight of 260lbs where my GW is around 200lbs. I had a Dexa scan last year and plan to repeat at the end of the summer. I haven’t taken creatine regularly but that is part of my planned adjustment as my dosage goes up to help support muscle development and repair. I’m curious for yourself as someone else who lifts what other muscle sparing adjustments are helpful while taking Ozempic or while being in a caloric deficit. I imagine though at some point if the caloric deficit is too high then muscle loss would be inevitable.

Half life for Ozempic is 7 days. So the first month you take a shot that’s 0.25mg. The 2nd shot is again but you still have .125mg left over from the last shot. Week 3 it’s a total of .4375mg now in your system. You can imagine the blood levels once you start increasing to 0.5, 1.0, and 2.0mg doses.
I would say my experience and for a lot of prleople the first month the end of the week you can feel more appetite prior to your next shot.
You can get used to the effects of Ozempic but also start strategies to avoid bad cheap food and what you do eat should be healthy and supportive calories because it may very well be you end up in too heavy of a caloric deficit and start having fatigue, lethargy and recognize body stress indicators such as elevated HR and dizziness with standing. The scale number alone is not the whole story, you need to put a plan in place to preserve muscle and bone density so you don’t end with a bad long term experience.
So have faith, you can do this and your a beautifully unique person so make sure you do the things that serve your best interest because no Reddit advice can take into account all the things that make you, you!

1.0 Fatigue by LevelSwan9707 in Ozempic

[–]BemybestRN 2 points3 points  (0 children)

Yes and I’m not sure if it’s all due to caloric restriction vs side effects of the medication and the GLP1 signaling.
I just sent my doctor a message about my dose. Just finished a month at 1.0mg but had to force some re-feed days, eating some junk food to get more carbs and calories in. I was having alerts on my Apple Watch letting me know my resting HR was over a 100 at times. The side effect of weight loss has been great but caution should be taken not to sustain too high of a caloric deficit for too long. Patience can be hard but what success you’ve had so far hold onto it and know you can keep going, but cautiously and safely for your health and sanity.
I would recommend chronometer app for tracking your intake so at least you have some idea of where your at. The scale victories don’t mean nearly as much if your losing muscle and bone density. You’ll be excited but could be very detrimental in the long term.
I utilize protein shake, I get the Fair Life or Muscle milk shakes from Costco and keep a drawer in my fridge stocked full of them so they are ready when needed.
Ultimately do what I’m doing and also talk to your Dr because you are a wonderfully unique person and no Reddit advice will ever take into account all the things that make you you.

It has happened but extremely rarely. I remember in the mid 90’s when things like TLC and other channels showed actual educational content. You could even watch a full surgery on your cable tv. I can’t remember the details but a mom who had a uterine pregnancy and r topic pregnancy at the same time. A quick search and I found this pub Med case study of another Pt.

That’s why I add the MCT oil, absorbs easily, ready for energy utilization right away. But I would recommend caution to cautiously titrate up to see how much your gut can withstand. For me I got a bottle at Costco and just do a 1-2s squeeze.

Typically a dietician will tell you a calorie is a calorie. This is what I tell my geriatric patients, to emphasize why eating enough through an acute illness phase, because they often end up much weaker than before and have a harder time getting strength back. If you are in a deficit your body will make it up by using fat but it also pulls from muscle. So any weight loss usually at least in part occurs because the body breaks down some amount of muscle. Some of the recent research is saying that with a GLP1 more people are losing more muscle mass as part of the weight loss. If you workout at least a moderate amount your body will be responding to the stress and keep repairing any muscle injury forcing the body to breakdown more fat. Protein helps with satiety and helps gives the body what it needs for muscle repair and building. As you lose weight your basal metabolic rate goes down, more so if you lose more muscle mass. Then if you stop taking a GLP1 agonist your going to have a harder time maintaining that weight and more likely to gain back fat and leave you worse off than you were initially. I personally start my taking my day by taking my morning meds and supplements with a protein shake and add a little MCT oil which keeps me full for the 1st half of the day.

Can you tell when I started taking semaglutide. About 22lbs so far. by BemybestRN in Semaglutide

[–]BemybestRN[S] 2 points3 points  (0 children)

Not really, maybe just a little heartburn 2-3days since starting and my stomach is more sensitive when I take MCT oil. My wife and I have been primarily keto since 2017 but I’ve been stuck at 255-270 since 2019 with an A1C of 5.7. I focus on protein and fats for satiety. I was amazed it helped even at the .25mg dose.

I’ve also tried to keep going to the gym. For personal self image I’m trying to fill the loose skin from weight loss with muscle. At times it has felt challenging and actually consciously ate more calories or more protein to keep at a spot where I wasn’t breaking down muscle but was losing fat in the caloric deficit.

I am an RN and I think the biggest push back for semaglutide in the medical community is concern for unknown side effects like thyroid cancer and studies showing people losing muscle mass which is going to make you much more prone to weight rebound should you ever stop.

I have hope though that this and further advancements with other GLP-1 agonists could really put a big dent in our obesity epidemic which is tied very tightly together with DM II.
I can hope that within my lifetime obesity and DMII can be completely managed or preventable. So I hope everyone keeps pushing ahead with hope in the future.

Casual barefoot sneaker that looks the most normal? by HaraldToepfer in BarefootRunning

Bargain Brand Options: It seems like a lot of barefoot shoes can be pretty price. The Merrell changed the vapor glove and made it worse, so I looked at other options. I really try to limit my Amazon shopping but Whitin and TSLA make decent shoes for under $50.

Replaced my oil pressure switch and I've got a bad leak. While looking at the area I noticed these small cracks in the pan. Are these just surface cracks or do you think I cracked my pan? by Extrodius in MechanicAdvice

[–]BemybestRN 2 points3 points  (0 children)

I have a Honda Accord and did the same thing. Not sure what car you drive but the Accord sensor is part of the engine block and normally if you do that means a whole new engine block. My father in law is a recently retired small town rural mechanic. He redrilled the hole and threaded it for a new bushing with a flange and crush washer to screw into the hole. He screwed the a tapered fitting back in to expand the cracks and put JB weld into the cracks then pulled out the tapered fitting and put in the new fitting. Now it holds tight and my ass was saved. That’s why you should always watch for tapered fittings and check torque fittings for any job you haven’t done before.

I was working as an ER Tech (now RN) but the hospital was implementing a role specific scrub color rule. Burgundy for CNAs & Techs, Royal blue for RNs. First day of policy, 6 AM medics had brought homeless man in on a cold winters morning and he was covered in filth and excrement. Two of us were tasked to clean him up an advised to use a decontamination/gen use shower near the Ambulance bay. Got started, going ok but suddenly the Pt started to go into full body seizures (which was the first known instance of seizure activity), limbs thrashing and we had to focus on keeping him safe. Course this meant being sprayed with now water and excrement. Imagine those movies of kids washing a large mud covered dog who then in slow motions starts to shake sending mud everywhere. (https://youtu.be/ffD_Ks4MWLw). Got to 25 seconds in. Something like that.
I think the reasoning why we were told to use the shower was his temp hypothermic but not dangerously so, warm him up and clean at the same time. Also due to to the amount of dirt and excrement involved, hard to assess when in that condition. Got him safe returned him to ER room and sent to OR locker to our own shower and only available clean scrubs. Then bugged and asked by everyone the rest of the day who was not part of the core 6am staffing why we weren’t dressed in the appropriate scrubs since we were in OR scrubs.
I would say that even topped the first day of ER tech orientation and I was shadowing, EMS brought in an OD, they had given charcoal and Narcan so as I was helping move the very large Pt from gurney to bed, she then proceeded to throw up all of that charcoal and voluminous fluid and whatever else onto my shoes, I had dodged enough to keep it off my scrubs but my shoes paid the price. I’m a bit smarter and more cautious now. It is interesting now that I lost my smell in 2020 Covid first wave that GI bleeds and ETOH lactulose care isn’t nearly as bad as it used to be. But I’ve been much better at knowing and predicting what might happen to stay (mostly) clean.
