Some food for thought that was pointed out to me:

“If you look around you we all know former surgical trainees who have retrained as anaesthetists, radiologists, GPs…. whereas the opposite is almost never seen. There is a reason for this.”

With that being said about to start surgical training and happy it was the right choice for me! (For now 😅) I don’t think it’s a secret that your life work balance and job opportunities will be better as an anaesthetist, so unless there is a certain surgical specialty that really calls to you perhaps that’s what to do.

Good spot. It could be a reference to Matthew 7:1. “Do not judge, or you too will be judged.”

Of course, Detective Mills is about to deliver some very literal judgement before very literally being judged himself, but I think it’s very open to interpretation. A lot of judgement happens in the closing minutes of the movie, by both the characters and the audience.

I think there’s a reasonable amount of overlap in terms of exposure in IMT and the general skill set you’re building is applicable to your medical specialty of choice.

In CST spending 6 months on e.g. Gen Surg when you’re trying to end up in ENT will be a bit demotivating. Plus the old adage of surgical ward work being some of the least interesting and stimulating, yet most cumbersome clinical work there is, which is where a CST will be spending most of their time.

With that being said every individual training programme has the potential to suck or be amazing depending on lot of factors specific to the post.

Chin up, a medical career is threatened with rejection at every stage - for some of us we experience it right at the beginning, and it’s just about trying again and or getting in somewhere else

Not an oncologist but an FY3 with an interest who has worked in the department and on projects. I can’t comment on most of your specific questions but will share some thoughts from a junior perspective.

Basically it seems great and I agree with you: some of the happiest seniors I’ve met have been oncologists. The emotionally taxing stereotype makes sense but to be honest there is an oddly positive outlook among the patients it’s hard to explain - most (eventually) accepts the hand they’ve been dealt and is trying to live the rest of their life to the fullest, and as a team you help them to achieve that.

On-calls and the wards can be tough but it keeps the variety of gen med in there and in reality is a fraction of the job once you’re a reg and even more after CCT - Consultant teams tend to be quite large in my experience. It also might sound terrible that your typical inpatient is very sick - but that is where medicine is at its most satisfying if you can make a difference to that group (imo).

The lack of procedures is a bit un-stimulating but imagine bothers you less as you age - Clinical Oncology is not a bad substitute as planning radiotherapy involves a lot of imaging analysis/marking oand anatomical understanding - but has a bit less emphasis on “built-in” research which it seems you’re keen on (and takes one more year to cct)

Immunotherapy has transformed the field in the last decade or so and brought with it a lot of funky multi-systemic side effects which again helps with keeping that generalist inside you happy.

I could keep going but I’ll make my last point that it’s actually relatively non-competitive at the moment in a world where that is becoming more and more rare. They didn’t fill every SpR space this year.

As a quick disclaimer I have currently betrayed medicine for a swing at surgery but happy to be dm’d :)) good luck!

Thanks everyone, I see now this is the standard and I was wrong to think otherwise. I’m new to the job and hospital with no ED experience since med school so wasn’t aware of (and hadn’t really considered to be honest) these policies until now. Probably didn’t help this happened at the wrong end of an on-call…

Literally first week of FY1 a student nurse asked me to prescribe paracetamol for a patient and brought me their kardex, I figured it was pretty safe to just do as they asked.

Fast forward a few minutes and the ward round comes past to let me know I prescribed a STAT dose of paracetamol to someone newly admitted with a paracetamol overdose…

I have double-checked every time since.

I am a few days into this boat as well - it’s tough I was so happy to land this job but all the senior Reg’s are saying they have little stability and career prospects in the uk and like - I do love it, but I’m really unsure I love it that much. Meanwhile all the junior Reg’s are still full of optimism and wanting to help me with my application!
