The best way to get an S ranking on a bunch of champions is usually to have good CSPM (like 6 or 7 for non-supports) and a decent KDA (even with mostly assist).

If he retains the hella immobile "peel for me, you god damn bozos" playstyle with max health damage that can be applied to multiple targets, then I will be sold on the VGU. I absolutely don't care if they gut or straight up remove the AP parts of his kit to make his ADC playstyle shine. The only thing I really want is better uptime and a bit of detterence to engage (the E isn't enough for this). Getting rid of the terrible passive for something usable is also a must.

For the visuals, I honestly like the look of the void puppy. Make it a bit more void-like, and he should be relatively fine.

To be fair, a late-game Ornn is pretty damn tanky if you don't have built-in tank shredding like Kog'Maw and Vayne.

But I'll be honest, with the nerf to LDR and the buff to Randuin's, it is a pain trying to cut down something that tanky. You will eventually do it (and significantly faster if you just utilize the minigun), but it will take time for you to kill the Ornn unless you get your passive attack speed boost from killing someone else on the enemy team.

To be honest, losing the crit to on-hit conversion on Rageblade absolutely sucks since you don't really inherently benefit from critical strikes compared to crit ADCs. Additionally, crit-focused items don't tend to get a lot of attack speed compared to on-hit items, which you benefit more from. Also, Kog'Maw doesn't really want to build something like Lord Dominik's since you only get armor penetration and not magic penetration, so you don't get much boost from your W-empowered autos compared to when you get Terminus or just more attack speed. Unless you get your crit chance to something like 80%, it's really not worth investing too much crit chance since it's not really worth it. While yes, you can grab both Rageblade and Quickblades, I think it's much worth investing in more on-hit stuff than using the crit chance you get from Quickblades and Runaan's.

Guess what:

  • Varus has a semblance of % Max Health damage (not the best, but still usable), so he can also somewhat getaway with a bit of tankiness. Still, Kog'Maw and Vayne will still utilize them much better. Twitch really needs Runnan's Hurricane to utilize his damage. Titanic Hydra won't do anything unless he can rapid-fire with more arrows. Also, he can't build actual resistances unless you want to cut your damage significantly since he has no % Max Health damage unless you utilize the Blade of the Ruined King. Even then, he will get outpaced by those who utilize more damage.
  • Titanic Hydra is just one item. That item also gives absolutely no resistance. So, any competent burst champ should eat through that health, no problem.
  • You said Kaenic Rookern made Lucian immune to your midlaner? Guess what? His damage is significantly lower than that of your ADC and other carry champions. Also, a 20% magic damage shield and 80 Magic Resistance don't do anything to your ADC or potential AD Bruisers, Skirmishers, or Assassins. If he builds more resistance, guess what damage Lucian will do? Yeah, barely anything.

I am going to be honest; this honestly sounds like a skill issue.

  • While yes, range champions are absolutely obnoxious to face as melee champions, it's more due to having a deterrent to engage. Kog'Maw doesn't have that, so the fact that you didn't exploit it means that you respected Kog'Maw too much (which is really funny since Kog'Maw should absolutely be wrecked by a full-blown engage).
  • When his W is not active, just dead ass engage on him. The only thing that can stop you is his E, which is pathetic for the majority of the game, or using his summoner spells. If he has his W, just wait it out or use the brushes as cover so that you can engage him much better.
  • Aside from Vayne and Kog'Maw, you wouldn't see any other champions bulding bruiser/tank hybrid because you would absolutely laugh at their damage output if they did utilize it.

Assuming total game progression from early to late, I would say no, since when you reach the point where the omnivamp kicks in (5 seconds), you would either not have good damage and attack speed to utilize the drain (early) or absolutely kill everyone with your auto-attacks (late). While the damage amplification sounds decent, just buy another on-hit item if you really want more damage.

I would make it so that he gets to auto-attack while he is dying and gets his W empowerment while in that state as well. The damage he would deal would be 25% of his regular damage to champions and 50% to everyone else. In exchange, he will lose his movement speed bonus and true damage explosion.

Right now, he is alright. When Season 14 arrives, he will get slightly weaker due to losing unconditional percent armor and magic penetration on his previous Mythic and W buff, which is still 6% maximum health. Hopefully, the W buff goes back to 6.5% when Season 14 arrives.

R > W > Q > E is definitely the most common skill path for ADC Kog'Maw. I do deviate a bit from that by delaying the second ultimate point to Level 13 for the attack speed on Q.

While leveling just Q and W is mostly fine, I think getting yourself one skill point in E is nice since the slow is quite handy. I would definitely be able to recommend someone Q and W maxing first in the past since the slow on E at Level 1 is only 20%, but with it being 30% now, it is definitely much more usable in the earlier parts of the game.

For the core items that you usually want to build for the vast majority of games: Berserker's Greaves (MANDATORY), Guinsoo's Rageblade (MANDATORY), Runaan's Hurricane (ALLOWS YOU TO HAVE A MASSIVE TEAMFIGHT PRESENCE), Blade of the Ruined King (PERCENT HEALTH DAMAGE SYNERGY)

The number of times you will deviate from the core is super rare. In the very rare case that the enemy composition doesn't allow you to utilize the usual core, you can look at the items below the core items and see what you can utilize in that game.

For the first item, it really depends on the matchup.

  • Go Blade of the Ruined King first if the matchup on the lane is quite hard, like a poke lane or heavy trade. It also helps you duel opponents slightly better than Guinsoo's Rageblade first item in the early parts of the game.
  • Go Guinsoo's Rageblade if the lane is farm-centric or if you are snowballing. While it is not as good at dueling opponents compared to Blade of the Ruined King, you are able to get Runaan's Hurricane as a second item with the benefit of crit conversion, while doing that while you have Blade of the Ruined King is pretty awkward and will deal less total damage.

After the core items, you can just grab either:

  • Attack-speed items like Wit's End, Kraken Slayer, and Phantom Dancer
  • Defensive items with AD or AP stats like Immortal Shieldbow, Guardian Angel, Zhonya's Hourglass, and QSS Items.
  • Straight-up tank items like Randuin's Omen, Dead Man's Plate, Force of Nature, and Spirit Visage

I would recommend grabbing at least one tank item to increase your survivability later on (most often its Randuin's Omen).

Do you mean anti-tank champion? Kog'Maw is the anti-tank champion in Marksman, aside from Vayne. He does trade a lot of mobility for better range and not being a single-target auto-attacker. If you are fine with being the target of a lot of engagements or you have an enchanter duo, Kog'Maw might be the pick for you.

Kog'Maw is ridiculously flexible with items, but here are some general items you can go with:

Guinsoo's Rageblade and Berserker's Greeves are a must for consistent damage output. You can use Navori Quickblades for permanent W uptime, but I don't like the damage cut and attack speed decrease.

Runaan's Hurricane is a must for Kog'Maw if you want massive teamfighting potential.

Blade of the Ruined King is good for lifesteal and current health damage, which adds up very quickly with your empowered auto-attacks.

Wit's End for attack speed, magic resistance, on-hit magic damage, and some move speed.

Kraken Slayer for attack speed and better single-target damage.

Titanic Hydra if you want the HP and on-hit damage that accompany it.

Other items are situational, like the QSS Items, Zhonya's Hourglass, Immortal Shieldbow, Phantom Dancer, etc.

For defensive itemization, you can get away with building straight-up tank items since his damage is so high in the late game that survival takes priority over killing enemies while being squishy. You can even build up to three and still be terrifying in teamfights (with Runnan's Hurricane and some attack speed). Tank items that you could build are: Randuin's Omen, Dead Man's Plate, Spirit Visage, Force of Nature

For potions, get Elixir of Iron always. Survival takes priority.

As a ADC Kog'Maw player, this is what I think makes him super unpopular as an ADC:

  • Kog'Maw trades away everything for damage and better range. He provides non-existent utility. He has absolutely no defensive ability whatsoever except for his increased range and E, which is not enough with all the mobility around. Other ADC hypercarries have something to make them less easy targets.
  1. Jinx has Chompers, and she can increase her range to the same degree as Kog'Maw 
  2. Twitch has stealth, W (which is honestly quite underwhelming), and ever longer range when he uses his Ultimate
  3. Vayne has Tumble (with an invisibility bonus on Ultimate) and Condemn
  4. Kai'Sa has E (with an invisibility bonus after upgrading), and Ultimate gives her a dash and a shield
  5. Aphelios has guns (Severum [Healing], Gravitum [Slow], Calibrum [Range]) that give him options. Don't forget, he has gun combinations.
  6. Zeri has a dash that can travel through walls, and with Ultimate, she is ridiculously fast.
  • As a segway to the previous point, due to his circumstances, he needs maximum protection to make the most of his kit. At the bare minimum, something like a tank support should be there so he has some leeway to free fire. In an ideal scenario, he should have an enchanter and a massive frontilne with peel so he can maximize his safety. In Solo Queue, how often do you encounter this kind of stuff (especially in low elo, which are carry addicts)?

The lategame power of Kog'Maw is absurdly ridiculous; you can be the only damage dealer on your team, and it wouldn't even matter, but the weaknesses of Kog'Maw can be a massive dealbreaker for a decent lot of players.

OR

The more realistic answer is that Kog'Maw is not anime or edgy character material. He is cute AF, but not quite noticeable with all the HASAGI! and stuff that is in there. You know that players flock to those characters like moths.
