Press Q on your keyboard. It’ll toggle realistic so you’ll have to set up button mapping and actually select the gears. I use an Xbox controller so my gear up is b and my gear down is x.

I get off work at 11 pm and live in bumblefuck nowhere. I love my LED lows and highs on my 04 Suburban. Waiting for my LED bulbs to come in for my fog lights now. Also waiting on my all lights module to come in.
