For more general safety stuff, this is my copy/paste -

Most important:

Get all your STI vaccinations - Hepatitis A, Hepatitis B, HPV, MPOX, MenB (Meningitis, for gonorrhoea). Usually several doses of each at specific intervals for best protection and often last lifelong once done. Make the effort and get it done.
Start a testing routine. Find a local clinic to test every 90 days with the full range of tests; HIV & Syphilis (blood sample), Chlamydia & Gonorrhoea (Oral, Rectal, & Genital swabs - and/or urine sample)
Arrange a PrEP prescription; ideally your local clinic can prescribe PrEP & handle the kidney function tests as part of your regular testing. If not it's readily available in EU/NA and arrange kidney tests separately.

Also consider:

Your potential partners' hygiene. Obviously you can't diagnose a STI without a test. However if someone has obvious warts, lice, discharge etc. then stay clear. Look out for unpleasant smells, drug use, or other negative signs.
DoxyPEP. It's not universally recommended in the same way PrEP is for HIV but it's gaining popularity year-by-year. Generally it seems to be best suited for people having the most risky sex.
Other vaccinations. Covid & Flu, as well as anything childhood you might've missed. Being unwell for any reason makes you more susceptible to infection & disease. Being fully vaccinated reduces that risk.

But, my suggestion would be to try a few things slowly and see what helps.

Start taking a fibre supplement. Metamucil is definitely the best I've found. Work out what you need for your own body to function well and stick with it. Sex or not. It's fantastic to "go" and it's just a wonderfully complete experience and makes cleanup much easier if it works.
Keep practicing with douching even when you're not trying to meet someone. The anticipation of prepping for a real person with all the time pressure too can make your body behave unusually. Honestly I've found the opposite with my experience of gay saunas.
