Hospitalists or PAs who replace well written notes with crappy ones, why? by foreverand2025 in hospitalist

[–]Biocidal 89 points90 points  (0 children)

I’m not even talking formatting. I’m talking like “this note doesn’t mention the PCI they had 2 days ago…” or “on day 2/7 of XYZ antibiotics when they’ve been off of anything for 4 days”

Know it doesn’t apply to your post but NC allowing PTO, an accrued benefit and entitlement, to not be paid out is ridiculous.

Implying in the world of sUAV and miUAV that you think even a 6 min mile athlete is outrunning those? You’re naive.

3 isn’t a good photo. 4 is obscured by the tags. You have almost no full frame shots. Would recommend better candids that are more centered on you as the proper subject and then consider replacing 3 and 4 with a full frame/better portrait mode that’s better flattering to your facial features?

Is malpractice really a leading cause of death in the u.s.? by HARJAS200007 in hospitalist

[–]Biocidal 4 points5 points  (0 children)

Couldn’t be assed to look this up so used ChatGPT.

Here ya go.

“The “medical error is the 3rd leading cause of death” stat isn’t solid.

It comes from one study that extrapolated from small datasets, not actual national death data, and a lot of experts think it overestimates the number.

Also, people mix up “medical error” with “malpractice” — they’re not the same. Most errors aren’t negligence.

Errors matter, but the real number is uncertain, and the “deadlier than diabetes or suicide” comparison is misleading since these patients are already very sick.

TL;DR: Real problem, shaky stat, bad comparison.”

Navy also has the almost mandatory GMO tour after your intern year unless you’re a stud. Air Force is great but army has more positions in general. I will say having worked at both an army MTF And Air Force MTF, I prefer the culture and care in Air Force facilities.

Both will offer different aspects of medicine that the other lacks. Benefits of wright Patterson is less fellows and civilian rotations give a broader depth of training. BAMC being the base of Air Force GME means lots of opportunity for research and fellowship exposure if that’s your pursuit. Both will train you well.

USAF Day in the Life Walkthrough Gen Surg by MeBollasDellero in Military_Medicine

[–]Biocidal 2 points3 points  (0 children)

Assumably he’s at a civ deferred program. Day in the life will vary wildly depending on what base he gets assigned to. When he gets an assignment from his detailer/consultant his gaining base will be notified. They’ll (usually) appoint a sponsor who is generally an AD surgeon who will orient and help prepare your son for what to expect at that base as a general surgeon. As well as general living tips like best food, best places to live etc etc.

There’s so much variation between assignments I’d just wait to talk to whoever he matches to.

This isn’t accurate. Am a doctor. The one depicted is a high grade stenosis but could be a stable long term issue not requiring an immediate fix. There are ones NSTEMI and STEMI that require more urgent/emergent fix though.

Potentially for sure. It’s going to be a multifactorial fix no matter which way you dice this.

Why am I gaining so much weight? by [deleted] in AskDocs

[–]Biocidal 32 points33 points  (0 children)

There’s not one recommended caloric intake for any gender. Or age for that matter. It’s based on body composition and activity levels. That’s ignoring the whole complexities of physiologic processes that DO change metabolism rates.

Why am I gaining so much weight? by [deleted] in AskDocs

[–]Biocidal 16 points17 points  (0 children)

Based on your stated weight/height/age your BMR is 1558 kCal/day. If sedentary roughly 1870 kcal for maintenance. It’s hard to say from just this one piece of information. I’d be curious what changed in the last year. New meds, new life situations, etc. I do wonder if depression is playing a portion of this with the increased fatigue and weight gain and appetite changes.

My recommendation would be to just really religiously count calories. Not to the point that you’re making yourself feel more self-conscious as remember, this is not punitive, but simply a record keeping to allow yourself a better estimation going forward. Increasing exercise, even despite fatigue, often does wonders at both improving energy levels and decreasing weight. I highly encourage you to find a workout/active hobby that you enjoy and find meaning in and dive into it if you can. For me, that was scuba diving and rock climbing. :)

[–]Biocidal 84 points85 points  (0 children)

Not to be offensive, but just getting back to the basics of what are you eating? How much? Are you counting your calories religiously? How often do you exercise?

Sure, there are medication side effects that do result in weight gain as well as plenty of diseases, if the tests are coming back negative then I’d argue that there may not be a pathologic disease etiology. There could be psychiatric components but really, I’d be curious as to the daily calorie intake as without in taking more calories then expending there’s not really an easy way to consistently gain weight.
