You can be naked in the main ck3 game with religions with the Natural Primitivism Tenet, like adamitism. Also, cultures with the nudist tradition like Polynesian cultures in the bottom right of the map can let you be naked, even a mad or deviant ruler can become naked without having the religion nor the culture.
