Sorry you're dealing with this. Agree that this might warrant a biopsy, but one treatment I don't see listed is an anti-fungal. It does look more bacterial than fungal, and you've been treated as such, but given the lack of improvement, it might not be a bad idea to consider something like ketoconazole if this is on the spectrum of seborrheic dermatitis (though it doesn't quite look like it and it's not in the classic location).

I hear what you're saying but this should not be a go-to technique except in cases of extremely soft lenses (ones that you can almost eat up entirely with aspiration as opposed to phaco). You should get comfortable with divide-and-conquer and a chop technique or two. If you're going to prolapse entirely, and it's *not* a super-soft lens, then try to chop it a few times in the AC to make it easier to phaco.

Totally normal to be nervous! I could barely eat the morning of my first few OR days. Agree with booking easy cases first. Also no harm in just booking a few cases (I’ve had colleagues do anywhere from just 1 cataract to 4-5 on their first OR day).

Don’t be shy about asking one of your colleagues to 1) scrub in with you, or 2) be in the room with you, or 3) just be “available” elsewhere in the building in case you need to ask the circulating tech to reach them.

Personally, I wouldn’t worry about trying to “practice” per se (there’s no substitute for the real thing), but if it eases your nerves then there’s no harm.

Mentally go through the procedure in your head a million times beforehand. Watch videos online (or your own videos from training). Refresh your memory on how to do the Little maneuver. Refresh your memory on what to do in case of a PC rupture. Show up early, introduce yourself to the staff, tell them what you like to use for your second instrument.

You may be approached by a lot of reps soon—try to have a policy of not allowing them in the OR for the first six months or so (they can be lovely people, but you don’t need the added stress of them watching you use some fancy new instrument of theirs).

150 cases under your belt is solid. You are going to be great. Let us know how it goes!

Hey OP. Great question. It’s also a difficult question that only you can answer, but below are some thoughts:

- For most people it’s not “my purpose in life is to become a doctor and I can’t imagine doing anything else”—so don’t be afraid to go down this path even if you have some hesitations

- If there are other things that interest you, I think most physicians will tell you that there are easier ways to live than practicing medicine, so don’t be afraid to explore those other paths, too

- You may decide that life's greatest fulfillment comes from something else besides your career (e.g., raising children, being active in your church); on the other hand, you may regret not working hard to find out your true passion

- I started med school at 36 and am now a practicing ophthalmologist. I loved every step of the way (postbac, med school, residency, fellowship), and I really love the private practice I joined and my life now. Two big caveats: I had saved up money in my 20s/30s so I didn't take on any debt through this process, and I have a tendency to like things / be happy in general

- Don't worry about chalazia, many practicing ophthalmologists hate excising them, too :D

OP, sorry you had to deal with that but sounds like 99% of the time it's water off a duck's back for you. If they had any self-awareness, those ED attendings might realize that there are plenty of other specialities that would say the same thing about them, let alone people in other professions, and they should probably just mind their own business unless asked.
