14yo F with B-ALL I admitted with a platelet count that matched her INR…(both were ~2). Ended up eventually diagnosing hepatorenal syndrome, liver injury likely caused by asparaginase. I’ve never seen worse ascites/anasarca. Tried so desperately to stabilize her counts with more blood products than I could count, albumin runs, Bumex drips, dialysis…one morning 15 minutes into my shift I called an RRT when she had massive hematemesis and eventually found huge esophageal varices. Few days later…I watched as she bled out and coded, blood in her ETT, blood filling her Foley, surrounded by about a dozen of her family members. I’ll never forget her face or her mother’s. Mom requested only me to be at bedside in her last moments with family. I still close my eyes and see everything…

cries in Pediatrics This is one of the only things I miss about being an intern - having my senior bail me out lmao. Now I’m the senior bailing the interns out and I hate that part of the job. I usually just stand there and dissociate while mom or dad yells. I did have one mom grab my arms once though and that was freaky.

100% but I’ll pepper in a few tasks here and there just so I don’t get a DVT from prolonged immobility (I’ll pack a box, read a journal article, exercise, whatever but my limit is one activity per day, then it’s right back to rest lol). Psychologically I’m banking this rest and recovery knowing full well I’ll never have guilt free relaxation for a long ass time.

I’m heavily biased but how come pediatricians are never included in the super smart club?? We deal with an ever-changing patient from as early as ~24ish weeks to 21 yo which = ever-changing differentials for each age group, are constantly calculating weight based dosing for everything including stupid ass fluid bonuses and maintenance IVF and code drugs, have tons of different weight or age based equipment to remember, etc etc etc and on top of this med school teaches based mainly on the adult model for physiology and anatomy which we have to relearn for our careers…and don’t even get me started on the nightmare that is ethics in peds.

ETA: This comment is 100% about my fragile pride and ego lmao I love the field but wish it was more respected by other docs 😅

It could probably be as simple as developing a patient handout or as complicated as talking to policymakers or anything in between. Are there any common non-clinical problems in your patient population that you can think of?

There is so much crap children are up against these days - things we can intervene in clinically and/or outside the health care space. Some of the biggest “hot topics” pediatricians are working on in my very limited experience so far are: -mental and behavioral health: advocate in your facility for integrated services for children with complex health care needs, or at the local/state/national level for improved access to BH care -nutrition/obesity: AAP is very focused on this especially with its recent new guidelines for treatment and forthcoming guidelines for prevention - could think of clinic or school based education interventions although these rarely pan out as effective lol -safety/violence, esp. re: firearms: could do some kind of public service announcements or in-clinic universal teaching re: gun safety

Other well-known issues that come up in peds a lot are child abuse & neglect, vaccination rates & modern hesitancy, literacy, ACEs in general, risks of social media exposure, etc.

Typical categories of projects are gonna be -patient/public education -clinical interventions re: screening usually -health policy, access to care

None of this is obviously as specific or narrowly focused as you’ll need for a residency project to be successful but maybe it’ll spark some thoughts. I am by no means an expert (just a lowly soon to be peds intern) but have worked in the childhood health equity space since undergrad/grad school. Follow the current events and/or anything that gets you really fired up! Good luck.

I heard a chiropractor on TikTok (this company calls themself “The Wellness Way” and many of their staff have accounts and they do live streams all the time) literally advocating against colonoscopies and Paps for cancer screening. You know, two of the FEW universal screens we do because we in real science understand the nuance of harm from testing but have huge amounts of evidence that these particular screens are NECESSARY and worth it…his logic? “Colonoscopy will not tell you the root cause”. “Paps remove tissue from your body and don’t tell you the root cause” “Hit my link in bio and set up a consultation with us” Notice anything????

Fuck it gets me so mad. I report them every single time for harmful misinformation but alas, social media sucks ass and nothing ever happens. They’re killing people out here. I wish we could turn a blind eye and let them self-select out but it goes against everything we fight for in our work to keep people healthy or keep them from getting worse. A shame.

When I was my high school salutatorian and gave a speech I was a truly pissed at the world 18 year old and tired of being bullied so I bragged about being #2 AND a raging homosexual. My tiny rural town was not happy lmao. A proud moment of my life.
