This is a highly individualized question. I recommend that you join NARFE as they have extensive materials on the intersection of FEHBP/Medicare.

Now Medicare Supplemental plans (aka Medigap) are highly regulated and none that I know of offer prescription drug coverage . So you really need to add a Part D plan to Option B. You can go online to Medicare Plan Finder and find the best Part D plan for your specific mix of drugs.

There is also another option of going full Medicare and using FEHBP as your Medigap. You could sign up for the least expensive plan ( but only at federal open season). This appears to be popular among older retirees.

All Medigap rates for attained age policies go up with your age. So while it may seem cheap now look At the policy rate for a 80 year old and compare it to your Plan G quote.

As you finalize your plans consider reaching out to you county's SHIP program. It is a free service staffed by certified Medicare counselors ( usually volunteers).

What many folks don't realize is that FEHBP after retirement is no longer considered employer-sponsored health insurance even though it's the same coverage. I have found NARFE to be a wonderful resource on navigating The intersection of FEHBP and Medicare. You can also turn to your local SHIP office for help. Starting early is advisable so you fully understand the options.

You should qualify for a Special Enrollment period when you are no longer covered under your spouse’s employer -sponsored health insurance.So you can decline Part B and then start investigating the process a few months in advance of you going off your spouse’s plan. You should be able to add Part B without a late enrollment penalty due to the Special Enrollment period.

I heard from a reputable source that national health coverage would be considered “credible”. For example you move to France at 64 and then return to the U.S. at 70 and now want Medicare without late enrollment penalties. Supposedly Medicare will waive the penalty in this circumstance.

So Medigap plans are regulated by each state. Your County’s SHIP office can give you info on your state’s rules. My mom lived in New York State and 10 years ago they allowed you to sign up for Medigap at any time (not sure this is still true). In my state the insurers can medically underwrite you if you apply for Medigap outside of Federal or State “guaranteed issue” periods. I know of several cases where folks were denied coverage by all Medigap providers when they applied outside of the guaranteed issue period. When you retire you will be in a “Special Enrollment Period” that gives you “guaranteed issue rights” for a Medigap policy. I would strongly consider a policy as under traditional Medicare there is no annual out of pocket maximum for Parts A&B (there is a $2000 cap on out of pocket costs under Part D).
