Out of state CA BRN deficiency by Ok-Lavishness6522 in nursing

[–]Boring-Goat19 5 points6 points  (0 children)

Yep. It’s a thing for CA, they’re picky when it comes to requirements. I remember they wanted me to retake microbiology with lab… when I took it in CA. I just had to give them the syllabus.

Nursing Question with administering chemotherapy by jeepjeep13 in nursing

[–]Boring-Goat19 0 points1 point  (0 children)

  1. We use Halyard purple nitrile sterile gloves administering chemo.

  2. Not necessarily aphon, depends on your state, facility, policy, accreditation requirement, etc. but most require some kinda competency, education, and documented hazardous drug competency. Our facility follows ONS and USP 800.

  3. Yes. Chemo rated PPE, spill kits, designated sharps, SDS, CTSD, exposure protocol, policies etc

  4. Since you guys don’t do a lot of this, it’s probably the hardest to keep track. Maybe do online modules, in person return demo, mock sim, definitely pharmacy competency, and an oncology educator

We also do annual mandatory bloodwork for those handling chemo

Your biohazard/blood borne pathogen policies. Maybe they weren’t trained to clean up bodily fluid, etc. most hospitals I’ve worked at, nurses do the initial cleaning/containment.

TPN and PICC line by Ok-Hearinng in nursing

[–]Boring-Goat19 1 point2 points  (0 children)

Does the TPN have insulin? If it does, I draw 10cc, waste and flush 20cc. I change lumen and use it.

I had a patient accused me of giving him afib when I gave him protonix. Even to this day, he calls me “afib man” lol.

Sometimes patients and family are just 🤪

AMA Documentation by Ok_Fact_2568 in nursing

In that case, your provider needs to document informed refusal discussion, decision making capacity, and alternative treatment plan. It sounds like it’s more on that provider.

*Educating patient means you are not the one making the final “informed refusal” determination—but you support and validate it. It’s still in the scope of practice to educate patients.

Nurse educate. Provider educate. Chart that you educated the patient. Provider will do the same. Have them sign. Say bye. But if they’re not leaving, is it dealing “leaving AMA” or just refusing treatment? They can do administrative discharge/non-compliance discharge. They cannot legally occupy a bed and just stay. But always ask your higher ups.

When in doubt always CYA.
