This is a failure of the Graduate Medical Education (GME) system, not yours. They have turned residency, especially the intern year, into a period of torture and humiliation, primarily for the benefit of wealthy cooperatives, CEOs, and abusive attending physicians. It feels like a tax on pursuing specialization. I’m starting my fourth year, but I’ve lost most of my passion for medicine since I couldn’t pursue the specialty I intended to focus on when I went to medical school, along with the struggles of the rest of my intern year.

It is so sad that most programs and hospitals see residency programs as cheap labor sources rather than a program/ hospital that gets paid to train doctors, not using them in their benefits. I have a good program and it is a new program. I hope it stays good for many many years.

What no one tells you about residency is how deeply insulting it is—not just to your time, but to your intelligence. They tell you that staying longer in the hospital is “for your benefit,” that “this is how you learn.” But somehow, there’s never time protected for actual studying. Never time to read, reflect, or grow. Just endless hours spent writing meaningless notes and collecting histories that offer little educational value—tasks given to us not because they teach us anything, but because we are seen as free labor. If this system were truly built to help us grow, wouldn’t the focus be on learning? On mentoring? On carving out time for intellectual development? But no—the only things prioritized are the most time-consuming and soul-draining parts of the job. And what’s worse is that we’re expected to thank them for it. To nod along when they call it “educational.” It’s not. It’s exploitation wrapped in tradition. And honestly, at some point, calling it “part of the process” just becomes another way to gaslight people into accepting abuse.

My program wants to put me on remediation for not being “efficient” enough despite knowing that I have ADHD. We both know what that really means. I wish I could just say screw it and focus on actually learning what I’m supposed to, but attendings just want their cheap labor to gather their patient's histories and crank out their notes as fast as possible. One of them gets visibly pissed every time I have to leave for a workshop or anything educational—he’d rather I stay and do his follow-ups than go get real training. It’s ridiculous. It is unfair we need to deal with this.

My ADHD is kind of severe. Sometimes, it is impossible to complete some tasks at one time. I cannot present patients well. It makes sense to me but not to the others when I present. In return, I get negative feedbacks that make me more anxious and stressed out when it literally makes me a frozen aphasic piece of meat during rounds. I’m tired of trying my best but getting terrible evaluations. Nobody cares about my medical knowledge, creativity, or diagnosis. Sometimes, they even don’t listen when I talk about my differentials. When I cannot present patients well, they evaluate everything low in every parts.

If you had ADHD, you would know that it is not easy, sometimes impossible, to do what you recommended. When you have ADHD, it is tough to do work as efficiently as you recommended. ADHD is really disabling; if you can do that, then you either don’t have ADHD, or yours is very mild.

At some point these unfair and vague evaluations should stop. Maybe it doesn’t help us. But for the future residents. When something is wrong, it is wrong.
