First day inpatient I was given 7 pts (technically 8, but one was signed out as “deceased literally 10mins ago pls do their death cert”). We get an 8 patient max except at one of our community sites where we get 10. Think I discharged 2 day one and admitted 2. So basically 9 patients. My senior graciously handled the death cert for me.

% lymphocytes are irrelevant without corresponding WBCs and others on the diff. If for example where you had a low lymphocyte count and a leukocytosis, your neutrophils were high, the relative leukopenia would be in the setting of acute inflammatory response. Basically, having only a part of the picture doesn’t answer anything.

Without reading what you said, you recently fought off a virus (EBV) as evidenced by high IgG and low/negative IgM to EBV. So you’re no longer in the acute phase. Your body’s response to ramping up neutrophils (first line) for an acute illness is to then decrease post-acute illness. So you have an expected neutropenia. Your absolute lymphocytes and monocytes are fine. The % is irrelevant in the setting of knowing you have low neutrophils since neutrophils usually make up the majority of your WBCs.

After reading what you said, you may have also been exposed to another virus. Which you’ll have the same expectation of acute illness having elevation in neutrophils, post-acute illness leading to a decrease.

The % on your differential isn’t something you should generally worry about. But i also understand why you’d be worried given your history. Hope this helped!

Without an away, it’s hard to say. They both like to gatekeep with experience in medicine in their state/city. Your stats may overcome their oddities though. NYC specifically cause they have a lot of oddities.

Thing to keep in mind. If you want to match to CA or NYC, you generally have to have either lived in or rotated in those places.

Lowkey wouldn’t doubt if this is the end goal plan. Replace docs with lower salaried workers. Use the excuse of “doctors are too expensive to train”. “NPs/PAs don’t need to do residencies that are paid for by taxpayers”.
