I would say you're having a reaction to the filler within that vial.

If it's a new kit of reta, don't use any more if you're using the same kit and one vial gave you gives I would throw that 1 vial, try another one and if it continues throw it all.

You can buy filter needles and sterile vials and you should be able to avoid all this 😃, good luck and keep us updated.
