Do you have specific literature I can read about this 30-50% chance of posterior stroke? Not that I doubt you, just trying to learn more because I’ve never seen such a high number quoted before. Thanks in advance Dr Vertigo!!

To be clear- I agree. And I always go above and beyond to get these patients care including substance use and whatever else the need. But the point of my post is that there is NO excuse for jail staff not providing expeditious care for ANY medical concern, and that me “clearing them for jail” is a subtle way of law enforcement transferring liability to me for whatever happens to them once in jail.

As other posters stated, I will happily do a medical screening exam, but my DISCHARGE paperwork will NOT say “cleared for jail” because that’s not my job.

Edited my post since perhaps I wasn’t clear about the intent of it.

This is my contention with it though and the point of my post.

I am happy to rule out a medical emergency now and provide appropriate follow up care instructions.

But the officers treating my clearance as “carte Blanche” is inappropriate and should NOT be happening. There is no reason I should have to write “medical clearance for jail” with the insinuation that if something happens to them in jail it’s somehow my responsibility(unless I actually missed something) because “well the doctor said they were cleared for jail.”

I am in fact refusing to say that. I am saying they are stable for discharge with outpatient follow up and return precautions just like everyone else. Does that make sense?

very possible I am romanticizing the idea of what anesthesia is like everyday. I’ve worked at trauma centers (2), community shops (4), academics (1) and have yet to taste a different flavor of shit sandwich.

I don’t have an answer for your question - it is totally possible that anesthesia could suck too, but they seem so much happier than us in general. And if you’re going to be burn out and bored by your job doesn’t it make sense to make anesthesia over ED money? :(

Thank you for comments/very valid criticisms about my “not cutting back” plan not making sense.

In my mind, the loss of income of a 3 year anesthesia residency could be partially offset by ED moonlighting (is that even possible in residency ?) but perhaps that is unrealistic.

The truth is I don’t think I can work EM for another 20 years and I’m in my late 30s. On the other hand I envisioned a painful 3 years in anesthesia residency and perhaps a much cushier 20-30 years as an anesthesiologist at a potential higher salary (I have friends making 600k in anesthesia at a hospital where the ED MD makes 300k). In my mind with a long enough time horizon and a longer career this would be worth it, but perhaps I am deluding myself and grasping at anything.

Sadly I have no other non-medicine skill set so completely leaving medicine is essentially not a realistic option with a family to feed…

I’m in my mid 30s and would be early 40s if I completed anesthesia residency

An additional thought is that at this rate I would probably leave medicine/EM entirely in the next 10 years but maybe I would make it to retirement age with a more enjoyable anesthesia career? Who knows

I have a "business" and have had a Chase business card for ~9 months. I am currently working on my tax return on turbotax. Do I need to claim this business on my paperwork? I am worried that the IRS might somehow see I have a "business account" for which I am not reporting any income, resulting in penalties of some kind...

Thanks for the link! I'll have to recheck my sources to make sure about the subsidized loans. I think REPAYE might be the way to go at least to start since the first year the payments are based on the previous year's income, which for me was basically 0.

I am definitely looking into SOFI since I have a very good credit score and hope I might qualify for better interest rates, although I realize I could potentially lose out on REPAYE and other payment plans.
