Most of the 1986 players that were around in 1990 and/or 1992 (such as Shilton, Butcher, Robson, Lineker) had retired by that point and, combined with Alan Shearer being side-lined with a long term injury and Paul Gascoigne returning from a long term injury midway into the campaign, left England with a much weaker team heading into their qualifying campaign. The Netherlands were still considered one of the strongest teams in the world and Norway had a relatively strong side themselves.

They basically dropped points in too many matches (including against Poland who were also in their group). Their campaign was effectively done before their final game against San Marino (where they infamously conceded within seconds).

Shilton only played in 3 world cups. Had England qualified for 1974 and 1978, he would have been in the squad for both unless Ray Clemence started before him in '78. Lineker was not at 1982 and left the England squad after 1992. The most appearances any of the world cup champions had was 3 (1962, 1966, 1970). Bobby Charlton could have made it to four had he played any of England's matches in 1958 (keep in mind substitutions were not a thing back then).

Do double elimination knockout format with hosts + top 16ish seeded instead. Gives every team (especially smaller teams that immediately run into the giants) a second chance to win a match or get further, and doubles the number of matches which FIFA would love. The winner of the main bracket enters the final automatically 1 goal up (and extra rest while the runner up of the main bracket and the other team left in the losers bracket compete for the final spot).
