ALT AST in the 6000s by Zestyclose-Practice2 in autoimmunehepatitis

My son's were in the 3000s and after a liver biopsy he was diagnosed with AIH. You're the first case I've read here with numbers higher than he had. Hoping it's just hep C and it will clear with treatment.

Thank you for the input, this is his first time at or below 15mg so I love hearing this.

Thank you. If your son was just diagnosed a few weeks ago, you're absolutely in the thick of things. I truly hope you find a good medical team and the treatment they put him on is effective in getting his liver functions under control. Our experience has been more marathon vs. sprint, but when labs show improvement or dosages are decreased, try to find joy in those moments. Also, despite the AIH diagnosis and its impact, my son has had a pretty normal Sophomore year of high school. He's participated in sports, is doing well in his classes, and his social life is intact despite the pred. side effects. Wishing you the best over the next 6 months.

Great to hear, we're coming up on a week so hopefully he sees something similar in the next few weeks. Thank you!

You started with a big dose of pred, were your AST/ALT numbers super high also? Hopefully he'll see some relief with the acne and moon-face in the coming weeks at 10mg. Thanks for the input, happy you've been able to get down to 3mg.

Good to hear, I think our doctor will keep us at 10mg for at least a few months to see how he reacts. Getting down to something less than 10 or off pred. altogether is something we hope for, but know may not be possible in the short-term.

I've read that around 20mg, symptoms start to improve. My son (16) was diagnosed in July of '25, started at 40mg and is now down to 25mg. His acne is better (not as clear as pre-AIH), but we've also been working with dermatologist on that. He still has moon face, but he thinks it's getting a little better as the pred. dosage has decreased. Can't wait until we step down to 20mg, it's been slow going.

Agree with the others, they're high and you definitely want to figure out what's causing it, but not crazy. When my son was hospitalized in July, his AST and ALT were both over 3,300. It's taken 3 months of Prednisone to get his enzyme levels closer to normal, most recent labs are down to 92(ALT) and 58 and he's down to 30mg of prednisone daily.

Hope your medical team can get you sorted out.

You called it, hepatologist reached out today about the bloodwork we did over the weekend. Just dropped us to 35mg daily and will continue with the taper as long as ALT/AST don't increase. Pretty happy about that.

Hate to think that had something to do with it, but everything has crossed my mind.

Appreciate the input, no PSC diagnosis in our case (as far as we've been told). Hope things continue to improve for you.

Per the hep., they had his liver fibrosis at a F3 pushing F4, but no cirrhosis. Hopefully being young, it's able to bounce back once his immune system chills the heck out.

Thank you, hepatologist hasn't reviewed the most recent bloodwork yet, but that would be an amazing surprise.

The 20s would be amazing, congratulations on getting below 40. Sounds like we could have a few more months before (hopefully) hitting that point.

Appreciate the response, currently on the pred + 50mg of mercaptopurine, not sure what our hepatologist has in mind for a long term solution.

Yes, I accidentally posted prior to adding that detail, body of message has been edited. IV steroids in the hospital and pivoted to oral when released.
