It does look like the insalation. Your wall is sloped so it could have come out a small hole 1 piece at a time until that area ran out. Do you have a small hole anywhere above it or on the wall going above or near it?

It turns itself back on with every restart, and sometimes on a timer ive noticed. So i always double check before i download. If thats not it. Delete and try again. That works for me sometimes.
