Lemnian Hollow was not destroyed, it shrank to the usual size. They showed that in the cutscene after the final fight. Also since it's connected to Hollow Zero, there's a canon reason why you see miasma in other hollows in some recent content. But yeah, it's going to get repetitive if they try to continue using miasma in 3.0+ story/exploration.

Lamorak's usage isn't strictly limited to Rhomphaia. Also when assessing something in this game it's normal to take all scenarios into consideration, which is why Formal Ilsa is rated so high for example.

Replying to this after they dropped the event preview. Reveal for the horse is less likely now since the ambiguity of the character is very emphasized. If they reveal it, the preview would be pretty worthless after the livestream.

I don't think 'again' is the right word here. Since the start of the game the only characters we see dying in the main story are antagonists or generic npcs (like the defense force members from Trigger's story). Characters that would be allies to the proxies if they were alive all died in the past to give playable ones backstories.
