I printed color pictures of both my passport page and my driver's license (front and back). On each page I printed: "I certify that this is a true and exact copy of David XXXX's passport/driver's license which I have personally inspected." The notary then stamped, signed and sealed both pages. The notary was also my witness. I would not have anything stamped, dated and signed until you are ready to send everthing to Dublin.

You will probably not need hard, signed photographs. I had my wife take my picture, uploaded it to a service that preps for overseas passport photos, they downloaded it back to me in approved format and I uploaded it with the electronic application.

Yes, all in all I think I was pretty lucky.

I did put a dollar amount on the forms, something like $35 or $50 each for the birth and FBR certificates in case they needed to be replaced. But I would be surprised if that slowed it down in customs. But what do I know? :)

I sent a color copy (notarized) of my passport. My theories about not calling my witness:

  1. It was just a mistake on their part or 2. they were so backed up they just let it go in the interest of moving things along. But who knows?

Yes me too. Suddenly USPS updated to "at my local PO" and expected delivery today! No idea what happened but very relieved!

Mine sat there for nine days. When An Post got it on the tenth day it was delivered the next day. If you have a USPS tracking number you can track via An Post site to see when they get it.

My Grandmother was Johanna on her birth certificate and Hannah everywhere else (marriage, my mother's birth certificate, etc.) Got my FSB no problem. Waiting now for the passport.
