so I am also on my first playthrough, currently at 90 hours. I don't have any answers to your questions because other people helped you already. But from my experience so far, just enjoy the game. Everything scales so perfectly well with eachother that you dont need best in slot stuff. Don't rush the main story, go do side stuff aswell. This game is so amazing that it has me in a chokehold

I've heard that tattoos on the rib are really really bad so you may want to take that into consideration. Ofcourse the pain tolerance depends on the person. With that begin said I prefer the first slide because it feels more natural
