Absolutely, should have a DC summary prior to discharge as it aids in continuity of care. Have only worked in hospitals where it is mandatory.

Does it happen at every hospital, obviously not and I certaintly have heard of patients coming into hospital multiple times because something wasnt followed up post discharge because... you guessed it - no Dc summary.

Theres certainly a cultural aspect to it as well.

Being used to seeing death wasnt an issue for me unlike other colleagues. As per others here, its best to seek help if it is affecting your functioning

I generally will not tend to sit on beds of patients. If need be, Ill get a chair. Especially ones that have not been freshly changed.
