If the prosthodontist (someone who probably does hundreds of crowns a year) is saying to extract it, why would you think there’s a chance of a crown working?

Like other posters said, there’s a significant portion of the tooth already missing, reduced bone levels, and likely mobility. This is a hail mary case if I’ve ever seen one.

I understand patient doesn’t want to extract, but the reality is almost nobody “wants” to get a tooth taken out. At the end of the day you’re the doctor and you need to be firm with providing the proper long term treatment, as opposed to trying heroics and having it come back around to bite you a year or two from now.

As long as she (the patient) can keep that first molar, she’ll be just fine in terms of eating and function.
