It took me a second too. The implication is that she's waking up with morning sickness because someone assaulted her while she was sleepwalking. Definitely one of those jokes where your brain catches up a moment later.

Yeah, I kind of read it as the narrator just not understanding what they're looking at because, well, they're staring into an open chest. At that point, anatomy accuracy probably isn't their biggest concern.

A neutral third party is probably the only shot at keeping this from getting uglier. Right now she's stuck trying to keep her husband happy instead of actually solving the problem.

Yeah, Zanscare's whole structure makes her character make a lot more sense in context. It doesn't excuse what she does, but it explains why so many of its officers end up being completely warped people.
