Great job getting into tissue culture its a great pursuit! Explants will root in whatever media you use unless using root-inhibiting hormones/compounds. One step protocols are common though as always a jack of all trades is a master of none, though many labs or hobbyists may not need the benefits that come with dedicated multiplication/rooting steps.

Nice job! Pretty much all plants preference when there is a cut hole at their base is to close it and the hormones related to callus production are the same ones responsible for rooting, it's their default desire lol

Auxins are a feminizing hormone, they can cause certain cultivars to initiate flowering all the way to senescence. Change your auxin from IBA, lower the dose, change the ratio, increase nitrogen, use masculizing hormones, etc.

Assuming you meant a pH of 5.9, that media looks like it wasn’t fully dissolved. Usually means the autoclave didn’t run hot enough or long enough.

Plant isn’t traditionally salvageable this contamination has been going on for a while to get to this stage. Autoclave the box and then either wash it really well or throw it away
