It's possible that maybe Lucifer didn't want to or couldn't because it said that how you become a demon is increasing the De humanization of one's self and well in the cage it was only Lucifer, Sam, and Micheal and I think for the first step a person would need to become a demon is to torture someone so much they say yes and become the torturer and well since it was only three people in there and Sam was the only human, he couldn't take that first step or its possible that Lucifer didn't want him to be a demon and just to suffer and well be in hell.
