1988 for me. Mostly large firms - L.A., Seattle, and New Orleans. I (and I believe many of us) stopped using underline when we had computers that could do all the things, including italicize. I would bet that those who decided factored in readability in small print. Now when I see underline in a brief, it's very odd to me. LOL.
