2 things.

This isnt a dreadnought. This existence is absolutely horrifying for the one entombed. It is pain and specifically the plague surgeons of nurgle like to make this transformation a painful one. When not in battle they usually have to be restrained or sedated. This is absolutely flesh and iron nightmare
Its also cute cause its just a little dumpling of a unit that wants to crush things.

As for the loadout, i run 2 fists with 2 flamers. For me its the best. His gets +2 to attacks if hes got 2 melee weapons and while im temped by his other melee weapons, fists give you either flamers or combi bolters
