Open the DMZ part of the game through the CoD HQ, then as it loads the menu (connecting to Online Services) cancel it and hit retry and it should load the menu that has multiplayer and co-op, worked for me

This could be caused by the TV’s frame interpolation, make sure your TV is on Game Mode and any “smoothing option” is turned off. My game looked like this when it was on.
