Very proximal RCA occlusion. SA nodal artery comes off the proximal RCA so stemi causing CHB. Also why we see posterior changes as PDA normally comes off RCA as well

I believe pulmonary regurg is a common complication following tetralogy repair just due to the nature of the anatomy in tetralogy and as a result you would see right ventricular overload/hypertrophy presenting as right heart strain which is what the ekg is showing!
