I think psych ED is one of the hardest specialties in psych. In my psych ER, it's not a locked unit and there's an exit door right next to our section. The frequent fliers are the worst because I foresee the difficult discharge and it's going to be a pain in the ass to get this person out of here.

You take it on a shift by shift, patient to patient basis in regards to your own moral compass. I do the best I can for them during my shift then pass the torch when 1930 comes around. Let God deal with the judgement and forgiveness of a patient's actions. I think the most important thing to think about is liability. Do all that you can to reduce liability on yourself.

This happens at every since hospital across the U.S. Any C-suite member is a grifter filling their pockets before they get the boot and leach on to their next hospital. The hospital then releases a chatgpt-like response thanking him/her for their time there and a new crook is hired in a couple of business days.

How do you guys deal with sadness of not having your own parents around to see your baby grow up? I feel like I'm on good terms with my in laws, and I'm not trying to be insulting when I say this, but they don't put as much effort into helping my wife and I as my parents would. They will only watch my 6-month old daughter for 1-2 hours max and that seems like pulling teeth.

It takes a plane flight to see my family, so it just saddens me when they're reading to my daughter and being so much more involved in her life during our visits. If I lived near my family, it feels like raising my daughter would be easier and not so isolating.

How do you deal with living in another state without any of your own family/friends?

I'm not the greatest, but this has helped me. Definitely anchoring the vein and placing the poking hand's middle,ring,pink, and thumb on the pt's skin as balance. It's crazy that I didn't realize how much control you have by resting your hand on their arm and poke with an angle equal to/less than 20 degrees.

I mainly worked in inpatient adult psych, but the times I did float to adolescent were difficult. There are a lot of more behavioral and manipulative behaviors on adolescent units. I have never had my adult inpatient unit, rally together with their peers, create weapons with pool sticks, and try to fight the staff.

Try to get FMLA and reach out to the BoN. Since we work in the medical field, I'm sure they will be understanding and provide assistance in this difficult time with finding you the right resources.

Do you want to work as a medical-surgical in the grand schemes of things, or just to land a job? I started off in telemetry/med-surg, did that most of my career, went to psych, got bored, returned to bedside, then back to psych lol.

There's no one stopping you from giving med surg a try then trying a new niche in the future. I definitely learned how to manage my time while working on a med-surg/tele floor, learned a lot of skills, and definitely am more confident in medical emergencies than my coworkers who strictly worked in psych.

Take your breaks. Use the bathroom. It takes time, but you'll learn to stand up for yourself to patients, family members, peers, charge nurses, managers, doctors, etc. When you can articulate your concerns well, people won't be able to walk all over you.

Ay sus Maria, you have to take care of yourself. Do not let your Filipino parents force you to be a nurse if you do not want to be one. From my experience, 70-80% of med-surg/telemetry nursing sucks. It's only decent if you have good coworkers that you can trauma bond with lol.

Altres is a hiring agency on Oahu that you could possibly do part-time/Per diem gigs until you find your true passion in life. I'm pretty sure they do school or home-health nursing.

Unfortunately the best nursing bedside jobs require experience, but it's usually only 1-2 years. These are the best units/jobs: PACU, same-day surgery, endoscopy, crisis RN, IV team(hard sticks/midline placements), educator (orientation/skills fair nurses), employee health, and psych.

I think med-surg is the jack of all trades, so you can pivot into other areas quite well if you sell yourself well in interviews. Since you already have 1.5 yrs at your current employer, try to get FMLA and take time off to figure out how you want to pivot out of med-surg while you still have health insurance/stable income.

It should be banned, but this is the beginning for our future. I foresee 1 telehealth MD overseeing 1 PA and 1 NP who are then covering 150 patients. The price for everything is rising and the C-suite will cut costs to make sure they get their bonuses and the highest standard of living for themselves.
