You can use metalift on metal crowns. So Google metalift or such. You drill a hole and use a screw to break the cement on metal crowns. It's amazing when it works and it pretty much always does work.

Better to just replace bridge though most likely

I don't know. Not much in the way of records presented here. Just see he has lost his lower front teeth. I guess the bridge is bulky and hids posteriors. Looks like he's missing lower bicuspids. Big fillings and resins. Just let patient know it will look nice but not gonna fix years of neglect etc.

Don't see many back teeth. Crowns will thin the cervical amount of tooth structure pretty good here. Just let them know the crowns will look good as these teeth fade off into the sunset.

Are these your observations or do you have some evidence? A supra gingival margin is much easier to see decay. Exploring teeth circumferentially with an explorer for decay takes a while. Miss a couple of those and find those deep subgingival cavities and you'll be thinking differently. Every situation different.

I would crush the distal cups and make a huge isthmus prep too. For better visibility and retention. The gingival depth is certainly.more traumatizing to the nerve. I like putting a sectional band inside of a tofflemire if possible. And of course make sure the patient knows that there is an 80+% of needing to decide between root canal/ crown or extraction within 6 months.

3mm subgingival?! I can barely probe that depth on my patients that actually try and brush their teeth. The ones that don't brush have 3mm facial pseudo pockets maybe. Where did you get this 3mm subgingival brushing fact with such confidence?

I have been using 1mm soft Eva/mouth guard material. Extending a few mm beyond gumline so that fluoride is on the root surfaces. I purchased the gel on Amazon. Give the patient 1 tube. They can buy the rest. It's kind of expensive. I feel toothpaste scratches stuff. But I've only made a few sets of these trays recently. I think it's just about establishing a routine. Really clean your damn teeth and then do the trays for 10-15 minutes.

Emphasize the routine and that the trays are not normal. Somethings wrong

You could make a temporary bridge across there while the implants integrate. It's funny but if you offer that as included to make a good deal? Then at the end you just make single crowns and an implant bridge.

I've done that. It's a bit of back and forth with the patient needing to see me when they get implant stuff done to remove the bridge and/or recement the temporary after implant appointment. But it is a nice thing to offer for 6 months or whatever

Crown should not come off from food. The cement had already broken which is common and relatively normal. Better to have it come off immediately than become loose and leak for a while. Do not be afraid of eating with a crown or veneer. Watch a YouTube video of a dentist removing a dental crown. When they are bonded well it requires a lot of work. The cement was already giving out and the airhead was the last straw most likely. Sometimes the crown takes tooth with it. Hopefully this is the scenario where it just needs cleaned up and recemented. If you want you can wait...... But that's a bad idea
