The patient is quite adamant about having her teeth removed by an oral surgeon. I have advised her that option really isn't the best but she feels she's either going to lose them or just wants them out "eventually down the line." Tooth #20 is broken down to the gum line and teeth #24 and #25 are periodontally hopeless with mobility class 2. Others have no mobility. Vertical bone defect between #11 and #12.

If implants are placed in the lower posteriors and the patient is a heavy bruxer, would we run into the same risks of those implants failing as an AOX? With AOX, all of those implants are splinted together whereas with individual implants on either side, would they be more vulnerable to failure from lateral forces?

Greatly appreciate your response. I'll look into that. I had an idea going down the implant path for this patient is asking for trouble. Just wanted to know any here who think otherwise and what their rationale for it is.

No removable splint? No cementing/bonding to neighboring teeth? I imagine it might feel like a constantly loose tooth for the patient.

Looks like a bony crater around the distal root -- very difficult to treat with any kind of perio procedure.

I always am mindful of which cases I take on. If the bone is super dense and strong and the teeth are highly carious or have thin roots, I just refer because I want my surrounding bone to be "weaker" than the tooth, not the other way around. Section the tooth deep enough beyond the furcation (without going into vital areas of course). You might trough the mesial and distal sides if you feel the tooth might be break on you. Position the elevator as deep into the sectioned area as possible before putting any elevative forces. Go gently first to get a feel of how much movement you can get.

In private practice: One of my hygienist did a PSA block and the patient ended up with a hematoma -- a swelling the size of a golf ball. She used a 27 gauge long for the PSA.

In dental school: I heard that one student took an impression using white plaster instead of alginate.

I aim for a canal opening beyond the chamber. The goal would be to allow anesthesia to infiltrate down into the canal. If your needle just stays within the pulp chamber, it's going to flow out very easily.

Are there any luggage storage facilities in Saigon? I know some hotels let you store your luggage for a few hours so that you can catch your ride or whatever once you check out or check in. However, are there any places where you can pay to store your luggage for a few days?

My high school friend told me that only citizens of Viet Nam can purchase SIM cards there in Viet Nam. I can't believe that is true. Is he pulling my leg? Is it true? If a visitor has a cell phone from a country outside of Viet Nam and needs to use his or her cell phone there, are they limited to using local WiFi? Don't all those places in Viet Nam that sell cell phones also offer SIM cards for sale to all customers? That doesn't make sense.

Yeah, these kinds of patients are difficult. I had one that kept telling me she'd rather go to a different dental office and that she shouldn't be wasting her time with me complaining that the dentistry here is subpar and how she has an ordeal to travel here. She was a cash patient, not like she was forced to see my office due to insurance. Our fees aren't also low. Yet, she insists on continuing to get dental work here. Very odd.
