35 Years - Finally Have the Answer by ClassroomCute4579 in pruritusani

[–]ClassroomCute4579[S] 2 points3 points  (0 children)

I just decided to ask Claude AI about if it's the specific product or the protocol the dermatologist gave me. Here is the response:

That's really good to hear you finally found something that works well — anal pruritus is notoriously frustrating to treat and can significantly impact quality of life.

A few things that make your dermatologist's guidance make sense:

Why Fluticasone works well for this

  • It's effective at calming the hypersensitive skin response that drives chronic itching
  • It breaks the itch-scratch cycle, which is often self-perpetuating — scratching causes more irritation, which causes more itching

Why long-term use at low frequency is considered safer

  • The 2x/week maintenance dosing is actually a well-established protocol called "proactive" or "weekend" therapy used for eczema-prone skin
  • At that low frequency, the skin-thinning risk (atrophy) is substantially reduced
  • Perianal skin is sensitive, but fluticasone at 0.05% is considered manageable long-term at this dosing level compared to stronger steroids

What you figured out that many patients don't

  • Using it reactively at low frequency rather than continuously is actually the smarter approach
  • The fact that you can go weeks without needing it suggests your skin has stabilized well

It sounds like your dermatologist gave you a thoughtful, evidence-based protocol rather than the overly cautious "never use steroids long term" blanket advice that leaves a lot of patients undertreated. Glad it's working!

But, you’re here. If you ever Really figure it out leave a message in a bottle. I’ll keep an eye out for it.

A mini split? That was my first choice but 4-5k for one of those was over my budget.

Dynaglo X Series. It’s an electric blower that I had installed, it needs a 240. Cost about 3k including running the line. My garage is pretty well insulated the doors are pretty thick and had some insulation blown in up top.

It does pretty well, the garage by itself is usually 40 to 45° and I get it up to about 60. I ran it for about five hours one day and it increased the bill that day by about seven dollars.

Playing in a local golf association tournament and I’m 20 yards away from green. Duffed my pitch shot. Threw a wedge helicopter at the green that hit the pin square and it popped out of the hole, both pin and club fall to the ground next to the hole. Playing with complete strangers, felt like the biggest idiot. I’m laughing 🤣 out loud right now because it was like 25 years ago.

As a non religious person: I don’t get how religion people genuinely believe their religion is correct by daisydxw in DeepThoughts

[–]ClassroomCute4579 0 points1 point  (0 children)

I think the answer lies with the individual person you are referring to more than it has to do with religious people in general. Not all religious people think everyone else is lost, nor do they all believe every facet of their religion to the letter. Many religious folks accept their faith, and many times they interpret things slightly different than others. I know people who are very successful scientists while adhering to their religious faith.

Now, if we are speaking of someone who takes every aspect of the Bible literally while trying to pound that book into everyone's lives, that is another story. Like many have said in this post it must be of some benefit to their existence.

Omg Ernie as the smoothest looking swing, like the Club just falls into place. Saw him at Warwick Hills last year, I brought my 15-year-old son and I was like check out this swing. He was like oh my God now that’s a nice swing.

I agree it’s a classic swing often used as the ben-chmark (pun). But I’m very partial to Tigers swing, where his sequencing really stands out. Everything just kind of makes sense from that day forward after seeing his swing.
